Recall of FreeStyle Libre 3 sensors

We wanted to make you aware of a letter that was sent to 1,365 Trust members who received recalled sensors for the FreeStyle Libre 3 continuous glucose monitors (CGMs). The letter is attached for your reference and will be mailed to impacted members on December 15, 2025. (Members will receive the letter with their plan’s logo depending on which prescription plan they are in, i.e. UHC logo for UHC MA-PD members, BCBS logo for BCBS MA-PD members, and Optum Rx logo for TCN, ECP, and HMO members.)

 

On November 24, 2025, Abbott announced a recall of multiple lots of FreeStyle Libre 3 and FreeStyle Libre 3 Plus sensors because they may provide incorrect glucose readings. Incorrect readings for a prolonged period may lead to wrong treatment decisions that may pose health risks.

 

Optum Rx notifies members who may be affected by these recalls. Members are provided information to help them identify if their product is being recalled and advised to contact Abbott—either online or via telephone—for a replacement. The letter also includes contact information for Abbott, the manufacturer, for more information related to the recall. Please be sure to share this with UBRs, retiree chairs, and other key stakeholders so that they are aware of the recall. Thank you for your time.
