FMLA claims are handled by Sedgwick (1-888-322-4462)Any absences or tardies MUST be called in 30 minutes prior to your start time.If you are going home utilizing FMLA the process is:

• Inform your supervisor that you need to go home early due to FMLA
• Call Sedgwick and notify them of your FMLA- home early.

• Call in "ILLNESS" to the absentee line (40 hours minimum)
• Have your PCP complete the reinstatement form
• Bring the reinstatement form to the employment lobby prior to your reinstatement date 
• Reinstatement hours are M-F from 7-11AM or 2-3:30 PM.
